Job Description

Are you are looking for an autonomous role, where you can help shape the future of community services?

Are you passionate about avoiding hospital admissions and caring for our patients in their place of residence?

Are you interested in joining us as a Specialist Practitioner (SP)?

Our Urgent Community Response (UCR) Service provides rapid assessment, diagnostics and treatment in a person’s home as an alternative to hospital admission. It is suitable for people who are in crisis and who need intervention within two hours to prevent hospital admission

UCR is delivered by a multi-professional team with access to specialist medical support where necessary to agree a management plan.

As a successful Specialist Practitioner (SP) clinician you will be embedded within the Community Teams which comprise of the Community Therapy Service, District Nursing Service, Community Matron service and Hospital at Home, however you will have the opportunity to work across many facets of Community services.

The role provides the successful clinician with a rare opportunity to work within our community services to test, inform and finesse the continued development of the Urgent Community Response service.

As part of a multidisciplinary team, you will work autonomously to provide a high-quality service to patients managing undifferentiated and undiagnosed conditions, using, developing and consolidating advanced clinical skills.

You will work collaboratively with the team and external providers, to ensure the right care, by the right person, at the right time and in the right place.

Your aim will be to enable individuals to maintain or regain independence or ensure a plan is in place for them to achieve their optimum personal potential and to prevent unnecessary hospital admission.

You will support a learning environment that fosters a culture of clinical expertise and act as a role model and mentor to advanced skills learners from all community services and teams.

Oxford Health Foundation Trust is part of the Buckinghamshire, Oxfordshire and Berkshire integrated care system (BOB), and been one of the national accelerator sites in the Ageing Well Programme in development of new NHS Urgent Community Response services.

You will be help play a key role in helping older people manage long-term conditions, making sure they receive the right kind of support to help them remain independent and live longer and linking in with our healthcare colleagues in the wider community.

You will be working alongside our Community Nursing and Therapy Teams, South Central Ambulance Service, General Practice and Oxford University Hospitals with the opportunity to work across many facets of Community services.

Our community teams are located across the county of Oxfordshire and this post on offer will align to the North East and Central Locality or a combination of both areas.

The service currently operates over 7 days a week, so flexible working into the evenings, over some weekends and bank holidays will be required. We are open to flexible and part time hours and the post is available part-time (minimum 30 hours per week) or full-time (37.5 hours per week).
